If I have a house under a sandwich L/O and the T/B dies in the pool or gets injured, who is liable?

Including Tenant/Buyers… which are nothing more than “Tenants”, you are “At Risk”. To mitigate the risk you have insurance coverage, and lots of it… It is a relatively inexpensive form of asset protection, when you have lots of assets to protect. If you have few assets, then it is considered “expensive” by most.

There are other forms of business planning such as LLC’s, Corps and the anominity of Trusts, that can assist in asset planning/protection.
