You might try Bill Mencarow’s “Green Board” on www.thepapersource.com. He sells lots of stuff like this for $1 or so @.

Most (all?)of major office supply stores have legal forms like notes, purchase/sale agreements, etc. for just few bucks. Office Depot, Staples, etc.

Finally, too, every decent law library at law schools, courthouses all over country, have legal form books which contain all kinds of legal forms which are freely copyable without restriction for personal use.

Robert, Check with a title company for a promissory note for NY but I think promissory note is very much standardized for all states. Check out www.dealmakerscafe.com
