Demand for Compliance or Possession - Posted by Mark

Colorado.
I have a tenant who’s given me the run-around for several months. He now owes 2 months of utilities exp. Their lease was clarified with an addendum dated and signed by all 12/18/01 stating they agree to pay for utilities including Nov, Dec. A warning letter sent out 12/20/01 for payment to be made (with a post mark no later than 12/24/01) and still nothing. I’d like to get rid of them for this and other reasons (jail time, police calls to residence etc…).
Question:
When using the “Demand for Compliance or Possession” form am I required to have it notarized? Who serves the notice?
Thank you.
