This particular eviction notice is NOT legal in the State of Ohio and will NOT hold up in court, since state law calls out very specific verbiage.
This is also the problem with many of the notices that you can purchase at Office supply stores. In particular, Office Max sells forms that will not be supported in municipal court.
Caveat Emptor. I emailed a Word document to Steve, which is a legal and acceptable form in this state.
For anyone who is interested, the form in Ohio MUST contain the following EXACT verbiage, preferably boxed and bold, and very conspicuous:
“YOU ARE BEING ASKED TO LEAVE THE
PREMISES. IF YOU DO NOT LEAVE, AN EVICTION
ACTION MAY BE INITIATED AGAINST YOU. IF YOU
ARE IN DOUBT REGARDING YOUR LEGAL RIGHTS
AND OBLIGATIONS AS A TENANT, IT IS
RECOMMENDED THAT YOU SEEK LEGAL
ASSISTANCE.”
